ABSTRACT

Objective:To study the structure of medical students' professional psychological quality and to compile a questionnaire.Methods:Based on the grounded theory research and semi-structured interviews, the concept and structure of medical students' professional psychological quality were constructed, and the questionnaire of professional psychological quality of medical students with qualified reliability and validity was compiled. The medical students from multiple universities in Sichuan were tested by convenient sampling. A total of 300 valid questionnaires were collected for item analysis, 400 for correlation analysis and exploratory factor analysis, and 400 for confirmatory factor analysis and reliability analysis. SPSS 25.0 was used to perform correlation analysis, exploratory factor analysis, and reliability analysis, and AMOS 26.0 was used to make confirmatory factor analysis.Results:The questionnaire of medical students' professional psychological quality consisted of three subscales: professional psychological personality, professional psychological ability and professional identity, consisting of 13 secondary factors and 61 items. The total Cronbach's α=0.947, the internal consistency coefficients of the total questionnaire and the three subscales were from 0.845 to 0.947, and the correlation coefficients between the three subscales and the total questionnaire were from 0.850 to 0.869. The three factor model fitted well (GFI=0.945, AGFI=0.906, NFI=0.936, RFI=0.906, IFI=0.957, TLI=0.935, RMSEA=0.070). Conclusion:Professional psychological personality, professional psychological ability, and professional identity are the core structures of medical students' professional psychological quality. Medical students' professional psychological quality questionnaire has good content validity, structural validity and internal consistency reliability, which can be used as an effective tool to evaluate the professional psychological quality of medical students.

ABSTRACT

ObjectiveTo investigate the diagnostic efficiency of FibroTouch, FibroScan, and acoustic radiation force impulse (ARFI) for liver fibrosis in patients with primary biliary cholangitis (PBC). MethodsA retrospective analysis was performed for the patients who underwent liver biopsy and were then diagnosed with PBC in Beijing Friendship Hospital, Capital Medical University, from September 2014 to October 2018, and the METAVIR scoring system was used to evaluate the degree of liver fibrosis and inflammation. Within 1 week after liver biopsy, FibroTouch, FibroScan, and ARFI were used to measure liver stiffness (LS); with pathological results as the gold standard, the area under the ROC curve (AUC) was used to compare the value of FibroTouch, FibroScan, and ARFI in the diagnosis of liver fibrosis in PBC patients, and related influencing factors were analyzed; Youden index was used to calculate the cut-off values of LS for different degrees of liver fibrosis. The Kruskal-Wallis H test was used for comparison between multiple groups, and P value corrected by the Bonferroni method was used for further comparison between two groups. A Spearman correlation analysis was performed, and a multiple linear regression model was used for multivariate analysis. ResultsA total of 68 patients with PBC were enrolled in the study, among whom 13 had F0 liver fibrosis, 15 had F1 liver fibrosis, 18 had F2 liver fibrosis, 12 had F3 liver fibrosis, and 10 had F4 liver fibrosis. LS obtained by FibroTouch (FT-LS), LS obtained by FibroScan (FS-LS), and LS obtained by ARFI (ARFI-LS) were strongly positively correlated with the degree of liver fibrosis (r=0.798, 0.782, and 0.742, all P<0.001). FT-LS had AUCs of 0.922, 0.881, and 0.926, respectively, in the diagnosis of F≥2, F≥3, and F=4 liver fibrosis, and the corresponding cut-off values were 10.5 kPa, 15.8 kPa, and 17.5 kPa, respectively; FS-LS had AUCs of 0.918, 0.878, and 0.939, respectively, in the diagnosis of F≥2, F≥3, and F=4 liver fibrosis, and the corresponding cut-off values were 10.1 kPa, 12.9 kPa, and 18.2 kPa, respectively; ARFI-LS had AUCs of 0.904, 0.869, and 0.928, respectively, in the diagnosis of F≥2, F≥3, and F=4 liver fibrosis, and the corresponding cut-off values were 1.45 m/s, 1.83 m/s, and 2.08 m/s, respectively. There was no significant difference in diagnosing the same stage of liver fibrosis between FibroTouch, FibroScan, and ARFI (P>0.05). The multivariate analysis showed that degree of liver fibrosis (β=0.399, P<0.001), total bilirubin (β=0.466, P<0.001), and prothrombin time activity (β=-0.195, P=0.020) were influencing factors for FT-LS; degree of liver fibrosis (β=0370, P<0.001), aspartate aminotransferase (β=0.450, P<0.001), prothrombin time activity (β=-0.303, P=0.001), and alkaline phosphatase (β=-0.187, P=0.042) were influencing factors for FS-LS; degree of liver fibrosis (β=0.489, P<0.001), aspartate aminotransferase (β=0.467, P<0.001), and platelet count (β=-0.188, P=0.028) were influencing factors for ARFI-LS. ConclusionFibroTouch has similar efficiency to FibroScan and ARFI in the diagnosis of liver fibrosis in PBC patients, with relatively high diagnostic efficiency for significant liver fibrosis (F≥2) and liver cirrhosis (F=4), and therefore, it can be used as a reliable method for the diagnosis of liver fibrosis in PBC patients.

Liver biopsy is the most sensitive and specific method for the diagnosis and staging of liver fibrosis, but its invasiveness and limitations have promoted the wide application of noninvasive evaluation methods for liver fibrosis in clinical practice in recent years. Various studies have proved the diagnostic efficiency of transient elastography (TE) in chronic liver diseases of various etiologies, such as viral hepatitis, nonalcoholic fatty liver disease, alcoholic liver disease, and autoimmune liver disease, especially in liver fibrosis and liver cirrhosis, and therefore, it has been incorporated into clinical practice guidelines in many countries. However, the diagnostic efficiency of TE in liver fibrosis is affected by several factors including aminotransferases, bilirubin, liver inflammation, and steatosis. Several noninvasive methods should be used in combination based on etiology to improve diagnostic accuracy.

ABSTRACT

Hepatic vascular diseases are a group of heterogeneous diseases which affect blood circulation in the liver. Although hepatic vascular diseases have a low incidence rate, they remain an important cause of portal hypertension. Patients often develop these diseases at a younger age, and misdiagnosis and/or mistreatment may greatly reduce their life expectancy. A good understanding of clinical manifestations, imaging features, and pathological features of hepatic vascular diseases is of vital importance in clarifying diagnosis and developing reasonable treatment strategies. This article introduces the pathological features of common hepatic vascular diseases and key points in differential diagnosis.

ABSTRACT

Objective To purify asprosin protein expressed in Escherichia coli expression system and to study its effect on cardiac function.Methods Coding sequence of asprosin was obtained from GenBank.Codon optimization was performed according to the codon preference of E.coli.After gene synthesized, recombinant plasmid was made.Asprosin was then induced and purified by Ni-affinity purification.The mouse model of impaired cardiac function was established by ligating and relaxing the left anterior descending coronary artery.30 mice were randomly divided into 3 groups: sham operation group (sham), cardiac dysfunction group (MI/R) and cardiac dysfunction plus injection of recombinant asposin protein group (MI/R+rAsp).The left ventricular function was detected by echocardiography to determine the improving effect of recombinant asprosin protein on cardiac function.Results After prokaryotic expression and purification, the purity of the target protein was higher than 95%, and the endotoxin content was less than <0.1 EU/μg protein, which was suitable for cell and animal studies.After the recombinant asprosin protein was given, the left ventricular function of the mice was improved significantly (P<0.05).Conclusions Asprosin acts as a myocardial protective molecule to improve cardiac function.

ABSTRACT

To cultivate innovative medical students is the inevitable trend to adapt to the social de-velopment in the higher medical education. Stomatology is a subject emphasizing on practical and innovation ability, so paying attention to the cultivation of students' application ability and innovation ability in the undergraduate stage, combining theoretical study and research innovation, can lay a good foundation for the study and work. In recent years, the College of Stomatology of Chongqing Medical University has created the scientific research atmosphere, established the platform of scientific research, attempted the undergraduate tutorial system, expanded the construction of teaching resource, and involved undergraduate to take part in the scientific research practice. The college has taken many active measures to train the scientific innovation ability of undergraduates, and has obtained good results. Some undergraduates have already possessed a certain ability to find and solve problems as well as independently think. They have also enhanced their awareness of reading literature and critically accepting knowledge and improved their comprehensive quality.

ABSTRACT

Objective To explore the expression and role of LAT 1 in mouse uterus on early placenta formation on day 8 of pregnancy (D8).Methods One hundred and twenty 6-8-week old SPF female Kunming mice were used in this study.Immunohistochemistry was applied to determine the localization of LAT 1 protein in the mouse uterus on D8 of preg-nancy.The ectoplacental cones (EPCs) were dissected out from D8.5 uterus, and then cultured in vitro with different con-centrations of BCH (2-aminobicyclo-(2,2,1)-heptane-2-carboxylic acid, specific antagonist of LAT1) and L-leucine ( substrate of LAT1) to determine the role of LAT1 during the EPC attachment and outgrowth .Results LAT1 protein was highly expressed in secondary decidual zone and also positively expressed in the mouse uterus on D 8.As a specific antago-nist of LAT1, BCH significantly suppressed the ectoplacental cone outgrowth , whereas L-leucine showed no significant effect on it.Conclusions LAT1 is expressed in the mouse uterus during early placenta formation and promotes ectoplacen -tal cone outgrowth , suggesting that LAT1 may promote the trophoblast invasion into maternal decidual tissue , and partici-pates in the early formation of placenta .

ABSTRACT

Objective To compare the clinical effect of preserving the ascending uterine artery in laparoscopic hysterectomy with traditional laparoscopic hysterectomy.Methods 112 patients with various kinds of uterine benign lesions performed by hysterectomy were randomly divided into the preserving the ascending uterine artery in laparoscopic hysterectomy group(observation group,n =60) and traditional laparoscopic hysterectomy group (control group,n =52),The operation time,the intra operative blood loss,the postoperative complications rate and the perimenopausal symptoms in two groups were compared as well as the levels of blood calcium and the levels of hormone before operation,the first month,the sixth month,the first year,and the third year after operation.Results The opreration time [(95.5 ± 22.8) win] and intraoperative blood soss [(81.3 ± 11.6) ml] of the observation group was more than those of the control group [(68.8 ± 7.2) ml] (t =3.772,4.418,all P < 0.05).The postoperative abnormal uniation and defection (1.7%) and fever incidence (1.7%) of the observation group was lower than those of the control group (11.5 %,13.5 %) (x2 =4.623,5.843,all P < 0.05).The estradiol level of the observation group began to fall in 6 months postoperative,and had significant differences compare with those of the control group 6 months,1 year,3 years after operation [(328.4 ± 13.4) pmol/L vs (304.8 ± 15.2) pmol/L,(311.3 ± 12.9) pmol/L vs (248.5 ±13.6)pmol/L,(258.3 ± 11.4)pmol/L vs (138.7 ± 12.7)pmol/L] (t =8.758,25.121,52.688,all P <0.05).There was significant difference in the level of follicle stimulating hormone (FSH) and luteinizing hormone (LH) between the two groups 1,3 years after operation(t =-90.540,-123.229,-14.757,-19.845,all P < 0.01).There was significant difference in the level of calcium between the two groups 1,3 years after operation(t =3.261,3.050,all P <0.05).The perimenopausal symptoms of the observation group was less than those of the control group(x2 =6.142,3.471,4.976,6.646,4.633,all P < 0.05).Conclusion Laparoscopic hysterectomy of preserving the ascending uterine artery is safe and reliable;the recently ovarian function of the patients with hysterectomy is to maximize the preservation in the treatment,the women's quality of life will be to improved.

ABSTRACT

Objective To explore the clinical value of high frequency ultrasound combined with serum tumor specific growth factor in diagnosis of thyroid cancer.Methods 155 patients with thyroid tumor received preoperative high frequency ultrasound and the detection of TSGF content,the histopathologic examination was given after operation.The clinical value of high frequency ultrasound and TSGF detection in diagnosis of thyroid cancer was analyzed.Results The sensitivity of high frequency ultrasound in diagnosis of thyroid carcinoma was 82.54%,the specificity was 81.52%.The sensitivity of TSGF in diagnosis of thyroid cancer was 79.37%,the specificity was 78.26%.The sensitivity of high frequency ultrasound combined with TSGF in diagnosis of thyroid cancer was 92.06%,the specificity was 93.48%.And the coincidence rate of combined detection was significantly higher than the two methods used alone,the difference was statistically significant (P < 0.05).Conclusion High frequency ultrasound combined with TSGF detection can make up the shortage of the two methods,which has important value in improving the diagnosis of thyroid carcinoma.
